Find a form completed by hand (delivery slip, warranty card, order pad), lay it flat and photograph the whole page, printed structure and handwriting both visible. Blank forms, crops that lose the printed labels and forms with others' personal details are rejected; up to 50 per contributor, from a global base strongest in Southeast Asia, the United States and Europe. Form types, languages and hands are uncontrolled. No field annotations, though printed labels make key-value pairing easy to annotate. Mixed print and handwriting on one page is a hard OCR case with no current public corpus: FUNSD is 199 non-commercial forms, NIST SD19 decades-old handprinted forms from 3,600 US writers.
